Incare, a division of HBTB, is a full-service skilled home health care agency offering a wide range of skilled services that can be provided in a patient’s home during an illness or following an injury. We are state-licensed and Medicare certified. We’re a team of health care professionals dedicated to helping our patients get better, regain independence and become as self-sufficient as possible, all in the comfort of their own home. Our success is driven by our ability to understand the physical and emotional needs of our patients.

Referral
Our referral process is made to be simple. Patients may be referred by:

- Community Service Programs
- Hospital
- Friends and Family
- Skilled Nursing and Rehab facilities
Just call, fax or email us your referral. Your call will be answered by an intake professional who understands your needs, can answer your questions and arrange for services.
Our Team
Our multidisciplinary team allows continuation of care through a patient’s lifetime. From admission to discharge and through multiple engagements, our patients are cared for by the same Incare professionals. We strive to become an integral part of our patients’ overall care and to be their advocates in the health care community. Each of our skilled staff is fully licensed, certified and insured. Our agency will bill the patient’s insurance source directly. The patient or responsible party will receive a bill only when a portion of a bill is not paid by insurance (e.g. deductibles and co-insurance payments). Home health care costs qualify as medical expenses and are tax-deductible, within certain limits, from federal income tax.
To Qualify for Our Home Health Care
- You must require nursing or rehabilitation services,
- You must be homebound,
- You must have the service ordered by your physician.
If you have Medicare insurance, you must be homebound to qualify for our services. This means that it is difficult and taxing for you to leave your home and that you need assistance to do so. Skilled home care services are covered by most insurance.
